HOME BASED CARE SERVICES
HSCS is a pilot programme which provides home-help support for adults with disabilities to continue to live in the community. The services include basic care (personal hygiene care, housekeeping and medication reminder service), therapy and behaviour intervention as required.

Community Group Homes enable persons with disabilities to live independently in designated HDB flats retrofitted with disabled-friendly features, an simulated environments eg. Thrift-Mart @ TUH
AD Hostels offer long-term and short-term accommodation and residential-based training for adults with disabilities who do not require institutional care, but are unable to live with their families.

SPECIALISED CASE MANAGEMENT PROGRAMME
Social workers and case management officers, the programme provides the following services:
- Co-ordinating and acquiring resources to support persons with disabilities and their caregivers
- Counselling
- Consultancy on disability information and issues
- Group⁄Peer Support
- Vocational⁄Motivational counselling and coaching
- Psycho-educational workshops to provide information on health issues and knowledge of self-care
- Caregiver support and training
